Nurse Practitioner Services provides healthcare consultations but does not guarantee specific medical outcomes.
Our services do not replace emergency medical care. In case of a medical emergency, call 911 or visit the nearest emergency room.

3. Patient Responsibilities

4. Payment and Insurance
Fees for services are due at the time of service unless prior arrangements are made.

5. Appointment Cancellations
We require at least 24 hours’ notice for appointment cancellations or reschedules.
Failure to cancel within this timeframe may result in a cancellation fee.
